Position Summary:


We have an exciting opportunity to join our team as a Social Worker.

In this role, the successful candidate under the Division Administrator for Cognitive Neurology and the Barlow Center Faculty Group Practice Manager, the Social Worker provides care planning to older adults with cognitive impairments at the Pearl Barlow Center for Memory Evaluation and Treatment, a NYS designated Center of Excellence on Alzheimers Disease. They will also perform psychosocial assessments for the NYU Alzheimers Disease Center.

Job Responsibilities:

Coordinates interdisciplinary information and recommendations into social work treatment plan and promotes timely care planning in the interest of patients/families;

Provide social work services including, short - term counseling, community resource planning, and crisis intervention;

Mobilizes community agencies and other support networks in the interest of patients/families;

Provides timely and effective psychosocial assessment and evaluations as required. Define problems and develops a relevant care plan;

Acts as a resource person for other members of the healthcare team;

Refer patient to outside community resources, classes, education, etc. as appropriate;

Assists in ensuring that patients receive services in accordance with established departmental policies and procedures through review of medical records;

Regularly participates in individual/group supervision;

Prepares statistical documentation of work in assigned areas. Demonstrates skill in presentation of psychosocial information in interdisciplinary rounds;

Partners with physicians, nurse practitioner and psychologists to ensure effective throughput;

Conducts orientation patient/family groups, educating and/or elaborating on information about diagnosis, treatment, options for long term care, etc;

Participates in interdisciplinary planning and unit/based interdepartmental meetings when requested;

Advocate on behalf of patients and their families;

Perform other duties as assigned.

Minimum Qualifications:


To qualify you must have a Master's Degree in social work. New York State licensure as a Licensed Master Social Worker (LMSW) or a New York Education Department-issued limited permit preferred.

Preferred Qualifications:
Previous work experience with older adults is required, experience with Alzheimers disease and related dementias is desirable.
Previous work experience in a health-related organization preferred
Experience documenting within a database system is preferred

Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $58,931.00 - $81,900.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
